RivMed’s Elder Care Program
Community-led Pathways to Care
This program is delivered by Riverina Medical and Dental Aboriginal Corporation as a member of the National Aboriginal Community Controlled Health Organisations.
To receive support, referrals must be made via a RivMed GP. Please speak to the clinic reception to make an appointment or call 02 69235 200.
RivMed’s Elder Care Program is available to eligible RivMed clients, over the age of 55 and living within the Wagga Wagga Local Government Area (LGA). To access this program, you will need a referral from a RivMed GP.
RivMed’s Elder Care Program delivers Aboriginal and Torres Strait Islander aged care support, connection, and coordination. Our RivMed Aged Care Navigators/Connectors help our Aboriginal community to ensure older Aboriginal and Torres Strait Islander people, and their families understand, navigate, and access the aged care services they are entitled to.
The program is designed to remove barriers and increase the number of Aboriginal and Torres Strait Islander people who receive care on Country.
Our RivMed Aged Care Navigators/Connectors deliver this program by clearly identifying the care our elders require to remain independent, within their own homes. This includes clarifying timelines and resources available.
Our team will continually monitor progress against the aged care plan and provide our wider RivMed team with the necessary skills and training needed to best support our elders.
How we support you
Our RivMed Aged Care Navigators/Connectors will help you to:
- Understand the aged care system, including identifying the level of care and services that you or your loved one requires
- Assist with making a My Aged Care (MAC) referral
- Link you with My Aged Care
- Link your nominated carer to the Carer Gateway
- Link you and your carer to Dementia Australia
- Link you with local providers and assist with changing service providers (if required)
- Reduce barriers to access care on Country
- Understanding your rights to access Aged Care

What you can access the
Elder Care Program for
RivMed Aged Care Navigators/Connectors can assist Elders and their families with:
- Speaking with My Aged Care on your behalf to arrange an assessment.
- Supporting you through the Aged Care Assessment.
- Help with finding local providers or assistance with transferring providers.
- Provide information regarding Rights, Responsibilities, Guidelines, Aged Care fees, and other Aged Care-related information so the elder can make an informed decision.
- Help you to understand your Aged Care Agreements.
- Following your Aged Care journey to ensure your care and needs are being met.
- Support you to resolve issues with your Aged Care Provider.
- Referrals to services outside of the Aged Care sector.
