Chronic Disease Management
At RivMed, we ensure that our most vulnerable community members are supported. Chronic illnesses are ongoing and persistent conditions such as respiratory diseases (including asthma), heart and circulatory diseases, high blood pressure, diabetes, cancer and kidney diseases. These conditions are common amongst Aboriginal and Torres Strait Islander peoples who are more susceptible to these illnesses, compared to non-Indigenous peoples.
Preventative measures can deter chronic conditions. However, the underlying risk factors make prevention and intervention complex. Through our comprehensive Model of Care, RivMed is committed to providing tailored support to our patients around prevention, intervention, and management of chronic illnesses.
Integrated Team Care Program
RivMed delivers the Integrated Team Care (ITC) Program for the Wagga Wagga region. The ITC Program aims to improve health outcomes for Aboriginal and Torres Strait Islander people with chronic conditions through tailored care coordination and support for self-management.
Who can access the RivMed Program:
- Have a chronic disease (diabetes, heart disease, kidney disease, lung disease, mental health condition or cancer)
- Have an ITC referral from their GP
- Have a Care Plan Team Care Arrangement (TCA)
- Needing support to access services or are experiencing barriers to accessing services.
- Needing support to self-manage chronic condition and coordinate appointments and referrals.

Booking Process
- Monday – Friday
- 9:00am – 10am: reserved for pre- booked appointments
- 10am – 12:30pm: open for on the day bookings
- 2:00pm – 4:00pm: reserved for pre- booked appointments
For earlier appointments, call us from 8:30am to see if any book-on-the-day appointments are available, if you would like to get in earlier.
Cancellation Policy
Please let us know if you need to cancel or reschedule your appointment.
Transport Support
RivMed can provide transport and taxi vouchers to help current patients attend appointments. Please ask reception for more information.
